What is double chin lipolysis?
Here a fat-dissolving product is injected straight into the fatty layer below the chin, known as the submental area, without surgery. Deoxycholic acid has been studied most. Your body already uses this molecule to break down fat from food. Once injected, it disrupts the outer membrane of fat cells, and over the next weeks the body slowly removes the damaged cells.
Less fullness under the chin can give a sharper jawline and a clearer angle between chin and neck. Only fat is affected: skin is not tightened and neck muscles are not changed.
Who is a good candidate for injection lipolysis?
| Often a good match | Another option may be wiser |
|---|---|
| Moderate, soft fat below the chin that you can pinch | Lax, drooping skin under the chin |
| Skin that still has good elasticity | Visible neck bands (platysmal bands) |
| Healthy adult with a steady weight | Fullness from other structures, for example glands or a thyroid swelling |
| Wanting to avoid surgery and able to attend several sessions | Pregnant or breastfeeding |
| Expecting change to come slowly | Trouble swallowing, a bleeding disorder or infection in the zone |
Cleveland Clinic recommends telling your clinician beforehand about bleeding disorders, infection where the needle will go, recent surgery or treatments on the face, chin or neck, any neck condition and problems with swallowing.
Injections or chin liposuction?
Each method reduces submental fat. Which suits you depends on how much fat there is, the quality of your skin, your time and what you prefer.
| Injection lipolysis | Chin liposuction | |
|---|---|---|
| What is done | A series of small injections | Minor surgery through a fine cannula |
| Number of sessions | Generally 2 to 4, around one month apart | Usually a single one |
| Anaesthesia | Local numbing | Local or general, according to the plan |
| Healing | Some days of swelling and numbness after each session | A compression garment and a few weeks of swelling |
| Outcome | Develops over several months | Seen earlier, keeps refining for months |
| Suits | Small to moderate fat with good skin | Moderate to larger fat, or when other surgery is planned too |
For visitors from abroad the number of trips often decides it. A course of injections means coming back several times, while liposuction is normally done in one stay.
How is the treatment performed?
- Assessment. Dr. Yazır examines your chin and neck, confirms that fat is behind the fullness, and looks at skin quality and neck bands.
- Marking. The zone to treat is drawn on the skin, frequently as a small grid, staying well away from the jawline where the marginal mandibular nerve lies.
- Numbing. Ice, a numbing cream or a little local anaesthetic is applied.
- The injections. A fine needle delivers small amounts of the product into the fat.
- Afterwards. Cold packs ease the swelling, and a review is booked before your departure from Izmir.
What does recovery look like?
| After the session | Typical course |
|---|---|
| Same day | Burning, stinging and swelling below the chin |
| Days 1 to 7 | Swelling, soreness, numbness and perhaps bruises; the swelling may be very noticeable |
| Weeks 2 to 4 | Swelling and numbness subside; the area may feel firm under the skin for some time |
| Around week 4 | Following session if needed |
| 2 to 3 months after the final session | Outcome judged |
Pooling randomised trials, a systematic review and meta-analysis showed that side effects mostly stayed at the injection site and were short-lived, often clearing before the next session. Pain, bruises, numbness, redness and swelling were reported most.
How do I plan my trip to Izmir?
| Day | What happens |
|---|---|
| Day 1 | You land, meet Dr. Yazır and, if appropriate, have session one |
| Day 2 | A quiet day; swelling under the chin is normal |
| Day 3 | Review at the clinic, then your flight home |
| Roughly 4 weeks later | Another trip for the next session, if planned |
Izmir Adnan Menderes Airport (ADB) is roughly 25 to 30 minutes by car from the clinic in the Egeperla building, Konak. Photos sent by WhatsApp beforehand are welcome; whether injections or liposuction fit you better is settled at the in-person examination.
What are the risks of double chin lipolysis?
- Swelling, pain, bruises, redness, itching and numbness
- Hard areas or small nodules beneath the skin
- Trouble swallowing, generally due to swelling
- Short-term weakness of the nerve supplying the lower lip, making the smile look uneven
- Skin damage where the needle entered (uncommon)
- A smaller change than hoped, requiring more sessions
In the phase 3 REFINE-1 trial, only a small minority of treated patients developed marginal mandibular nerve weakness; it was mostly mild and recovered fully. Precise marking and careful technique lower this risk.
What does the evidence say?
Randomised, placebo-controlled trials and the meta-analyses built on them show deoxycholic acid reduces fat under the chin more than placebo. A 3-year follow-up study found that most patients who responded kept their improvement. A 2023 meta-analysis, however, judged the certainty of the evidence low to moderate and pointed out a possible industry-sponsorship bias in all qualifying trials. For that reason this page talks about realistic, gradual improvement and not about guaranteed outcomes.
How long do results last?
Treated fat cells do not come back, so the change usually holds for a long time, as the 3-year REFINE follow-up showed. The fat cells left behind can still grow if you gain weight, which is why keeping your weight stable protects the result.
Why does a surgeon's assessment matter?
A full look under the chin does not always come from fat. Lax skin, neck bands, where the chin and jaw sit, and structures such as the salivary glands may all contribute. As an ENT and head and neck surgeon whose practice focuses on facial aesthetic surgery, Dr. Yazır offers both injection lipolysis and chin liposuction, so the decision can follow your anatomy rather than a single preferred technique.
