What are lip lines?
Perioral lines, which people also call vertical lip lines or smoker's lines, are small upright creases that run across the lips and the skin just above the upper lip. According to Cleveland Clinic they tend to show up around the age of 40, sooner in smokers and in people with sun-damaged skin. A common complaint is lipstick seeping into them.
These lines are not the same as nasolabial folds, which run from the nose down to the corners of the mouth, or marionette lines, which go from the mouth corners towards the chin. Those are treated differently; you can read about them on the nasolabial fold fillers page.
What causes lip lines?
Around the mouth the skin is delicate and moves almost all day. Over the years it loses collagen and elastin, and each time you talk, sip or pucker, the ring-shaped muscle around the lips folds it again. Factors that add to this:
- Ageing and the genes you inherit
- Smoking and frequent lip pursing, such as using straws
- Sun exposure
- Thinning of the lips themselves, which leaves the skin above with less support
Who is a good candidate for lip lines filler?
| Generally a good fit | Wait, or talk it through first |
|---|---|
| Fine or moderate vertical lines over the upper lip | Very deep, etched lines that may also need resurfacing |
| Lipstick running into small lines | An active cold sore or other infection near the lips |
| Lines surrounding lips that are thin | Being pregnant or breastfeeding |
| Wanting a subtle change without surgery | Expecting the lines to disappear for good |
| Not smoking, or willing to cut down | Recent laser or peel treatment here (timing is planned) |
Which treatment options are available for lip lines?
Cleveland Clinic mentions several options that avoid surgery: fillers, low doses of botulinum toxin, peels, laser treatment, microneedling and PRP (platelet-rich plasma). Which is right depends on the depth of the lines, the condition of your skin and what you are hoping for.
| Approach | What it does | Usual duration |
|---|---|---|
| Soft HA filler | Fills each line and supports the skin from below | Frequently up to about 12 months |
| Low-dose anti-wrinkle injection | Calms the muscle that folds the skin | Up to about 6 months |
| Skin resurfacing (peels, laser or microneedling) | Refines texture and fine lines | Variable; often several sessions |
| Lip filler | Gives back lip volume that holds up the skin above | Usually 12–18 months |
| Surgery (such as a lip lift) | Changes upper lip height and form | Long-term |
In a randomised study from 2015, treating the lips together with the small lines around them using a soft HA filler reduced perioral lines, and the improvement could still be seen six months afterwards.
How is the treatment performed?
- Consultation. Dr. Yazır looks at the lines with your face relaxed and moving, and checks lip volume and skin quality.
- Numbing. Anaesthetic cream is applied, and the filler normally contains lidocaine.
- Injecting. A soft, flexible HA gel is placed in tiny amounts just beneath each line, often using a fine needle.
- Evening out. The skin is lightly massaged so the filler spreads evenly, then a cold pack is applied.
Since this part of the face is in constant motion, soft fillers made for dynamic areas are usually preferred. The exact product is chosen during the consultation.
How should I prepare for the treatment?
Little preparation is needed, but these steps can cut down bruising and make the day smoother:
- List every medicine and supplement you use – blood thinners, aspirin, fish oil and vitamin E in particular – and do not stop anything prescribed unless your doctor tells you to.
- Mention allergies, a tendency to bruise, earlier fillers (with the product name if you know it) and any skin conditions.
- If you get cold sores, tell the clinic beforehand, because injections near the mouth can bring one on.
- Skip alcohol the day before and arrive with clean skin and no make-up on the area.
- If you would like a first opinion before travelling, send photos over WhatsApp.
What does recovery look like?
| After treatment | Typical course |
|---|---|
| The first 24 hours | Light swelling, redness and pinpoint bruises; no make-up on the needle points, no straws, smoking or hard exercise |
| Days 2 to 4 | Redness and swelling fading |
| Around 1 week | Bruises have mostly cleared |
| Around 2 weeks | Result reviewed; a small top-up planned if necessary |
How do I plan my trip to Izmir?
1–2 days is usually enough. The clinic is located in the Egeperla building in Konak, about 25–30 minutes by car from Izmir Adnan Menderes Airport (ADB).
| When | What happens |
|---|---|
| Day 1 | Consultation and injections |
| Day 2 | An optional check; you fly home once Dr. Yazır agrees |
| Around 2 weeks on | Follow-up with photos sent over WhatsApp |
How long do the results last?
The effect is not permanent. According to Cleveland Clinic, filler used against these lines may hold for as long as 12 months. A 52-week randomised study examined a resilient HA product in people whose dynamic perioral lines were moderate to severe: at eight weeks roughly 81% of treated patients responded, compared with roughly 8% of untreated controls, and after one year 66% still did. Daily sunscreen and staying smoke-free may also make the lines less visible in the long run.
What are the risks of lip lines filler?
Most reactions are mild and brief. Risks to be aware of:
- Swelling, redness, soreness and small bruises
- Little visible bumps or a faint ridge where filler sits too close to the surface
- A bluish shadow where the skin is thin (Tyndall effect); hyaluronidase can correct it
- A cold sore coming back
- Infection or nodules that appear later
- Vascular occlusion – this area lies close to the upper lip arteries; severe pain or a pale or dusky area of skin needs treatment straight away
When randomised trials were pooled in a systematic review, HA filler side effects were mostly brief reactions at the injection site that cleared by themselves, and serious events were rare.
Why do patients choose Izmir for lip lines filler?
Only a brief visit is required, which suits a trip to Izmir well. Dr. Yazır is an ENT and head & neck surgeon whose practice focuses on facial aesthetic surgery; he examines the lips, the skin over them and the rest of the face as one unit before suggesting filler alone or alongside other treatments. Prices are also lower than in the UK or USA – the estimated comparison is shown below. You may also want to read about lip fillers and our dermal fillers overview.
